What is IRMAA?

IRMAA stands for Income-Related Monthly Adjustment Amount. It is a surcharge added on top of your standard Medicare Part B and Part D premiums if your income exceeds certain thresholds.

The key thing to understand: IRMAA is based on your income from two years ago. So your 2026 Medicare premiums are determined by your 2024 Modified Adjusted Gross Income (MAGI) — the number on line 11 plus line 2a of your Form 1040.

If your income has dropped significantly since 2024 — due to retirement, reduced work, or a life event — you may be able to appeal your IRMAA determination and lower your premiums now.

IRMAA Appeal Process

If your income has dropped since 2024 due to a qualifying life event, you can ask Social Security to use a more recent year's income instead. A successful appeal can reduce or eliminate your IRMAA surcharge immediately.

How to Submit Your Appeal

  1. 1Complete Form SSA-44Download and fill out the Medicare Income-Related Monthly Adjustment Amount Life-Changing Event form.
  2. 2Gather DocumentationCollect documents that prove your qualifying life event. You can still appeal without proof by attesting to the event.
  3. 3Submit Your AppealSubmit online at SSA.gov, or mail your completed form and documentation to Social Security within 60 days of your IRMAA notice.
